Joint a cylinder to a disc shape

I have a cylinder that I ideally want to loft or connect anyway to a disc.
My design is

The connection should go from the end of the cylinder to the rounded edge of the disc.
Does anyone have ideas of how to achieve this.

    Here's my take on it. Make the transition more gradual by increasing the separation between loft profiles. Add a fillet and split the circle into a matching number of segments in order to make the loft look symmetrical. I've added a couple dimensions to the Loft Circle Sketch that can be tweaked to get a different look. Use Normal to Profile end conditions in your loft. And lastly, apply vertex matching if your loft gets "twisted".
