Cutting holes into a cylinder at 45 degree angles from Top and Right panes

I need to add some holes to a cylinder and I am having a hard time figuring out how to sketch those on when I cant seem to get a sketch plane created at a 45 degree angle. The holes need to be located at 45 degree angles from the rail sections, and while I think I found a very dirty work around by sketching on the front face and extruding the feature back then down, that doesn't really accomplish what I am looking for.

Ideally I would like to sketch one hole the shape I want, then use the linear and circular patters to spread those to the rest of the model so I can update the size and shape of the hole quickly.

Is there any way to do what I am looking for?
