CAD Basics Survey

I have recently completed the 4 courses for CAD basics and I am trying to get a certificate for it; however I am not sure where to find the survey I have to take before I can get the certificate as it doesn't appear underneath the courses. Any tips on how to access it or where to look?

First, check if you have all the green check marks and green circles as above.
Second, check if you have really finished all related courses. NOTE: Don´t miss to give feedback, otherwise you can´t recieve your certificate.
