Making a concave surface

Im trying to figure out how to make a concave surface in a cylinder.  This should be very simple yet everywhere I search it seems so complicated and I still can't figure it out.  Can somebody explain in simple terms or a video of how to make a concave surface in a disc?

You can make this by revolving a rectangle where one side is an arc. There are plenty of ways to do this, but this is probably the simplest.

I have a solid disk 33.5 mm wide by 10 mm high and I want one end to be slightly concave.
