Plastic Threads - American Flat 6 - Odd behavior

Hello,
I am designing a part that will be made in steel. I wanted to get a 3D printed version as a sanity check but the threads are not coming out OK.

In KA-Drain_V8 Parts, I have a Drainpipe. on the outside wall, there are two surfaces - a 39mm OD and a 37mm OD. Both threaded surfaces
When I add threading the 39mm surface, it weirdly adds a surface below it. Am I doing something wrong?
I did not have this issue using Onshape's cosmetic threads not with the Thread Creator tool (but with that, the threads did not work)
