sheet metal flange tool not making extrusions on both ends of partial flange

Greetings,
I am using the sheet metal tools to make a skin for a device (4 parts, one per side). I have used the flange tool in three places on all four side panels. On two of the instances the tool only puts a hole at the top of one of the little spacer gaps cut at the end of a partial flange (see attached). I've redone it several times but it does the same thing. I can make a sketch over the hole and make the extrusion myself as a work around but kind of a pia.

I have had problems like that .....usually some intersecting out of plane issues. post a link0
