Sketch generation.

I am new to Onshape so my questions are basic. I have watched the various tutorials on extrusions and created a long rectangular cubic form. I am now putting holes thro it from various planes just to gain experience.
Some times it works fine and then i find that I select a plane but cannot generate a sketch which appears in red. I am doing something wrong but cannot understand what.
On a related problem. When do you use the various extrude options eg. Thro and to next face ? Thro seems to work.

You have to leave the sketch dialog open while sketching. Clicking the green check mark closes the sketch and the system will assume you are done sketching. If you double click on that sketch, you'll notice that no sketch plane is selected:
So, you can double click on sketch 4, select a planar face or construction plane, and then start sketching.
There are example of all the extrude end conditions and what they do here:
