Loft Hole

I am designing an adapter that transitions from a square opening to a round cylinder. I am trying to create a loft with a 1 cm wall thickness. I have tried creating two separate lofts one solid loft and one loft to remove the internal material but I still end up with these holes. Has anyone run into this issue before or…
