Trackpad

Is it possible to design with just the trackpad or will I need a mouse?

It's very possible, but a mouse is much easier in my opinion. You'll want to use the Onshape defaults for the controls in order for the trackpad to work.

I still prefer to use a mouse and 3DConnexion Spacemouse for serious sessions, but I use just a trackpad very often and it's totally doable. I'm on a macbook pro and the trackpad is pretty big which helps.

It's definitely possible to use only a trackpad. I've exclusively been using a trackpad on my MacBook Pro for Onshape for the past few years.
