Design Help - Make Into One Part

Hello,
I was hoping I can get some help… Can someone please help me turn this design into one part/piece? It is currently a sheet metal design one bend with a tab at the top.
Thank you in advance.

Use the "tab" feature instead of extrude2
I can't get it to work… sorry I don't have much experience with the Tab feature. Thanks again
Look under sheet metal functions for tab. A little reading in the help file will explain.
I got the tab feature to work… sort of. I am getting a "Collision In Sheet Metal Flat Pattern" error. I haven't been able to fix it after trying for a couple days! I believe my measurements are correct. Any help would be great…

The flat view does show where the problem is:
