challenging milk carton design

Hi community, I am trying to model this milk carton in Onshape using the sheetmetal functions, but am struggling to do the top. On two opposing sides, the rectangular face bends into a triangle by following the 2 creases. This part really confused me because I am unsure how to do this in Onshape. Any pointer would be appreciated.
