loft pinching

Greetings,
I use the loft tool frequently and find myself spending waay too much time trying to keep my lofts from pinching. If I put circular profiles on, and normal to, both ends of a simple single or two curve spline, why do I always get a waist and or a twist in the middle section? I shouldn't have to make a dozen intermittent profiles - that's why I'm using the loft tool. Seems like a pretty simple thing.
