Offset failed. One spline generated multiple offset curves.

    If the minimum curvature is smaller than the offset (in the direction you're going) then it will fail. I would either clean up the original curve to reduce any sharpness or consider starting with the innermost curve you want and offsetting outwards. You can check the minimum radius with the curvature tool (shift-C).
