It happens to all of us. Caused either by "oops, I forgot to select a surface" or a tiny sliver face that you didn't notice until you put "highlight boundary conditions" on.
Are there some particular, difficult-to-diagnose examples of Enclose that anyone can share?

Not sure if this is the sort of stuff you mean but it was pretty difficult to get a solid out of this imported model!
