Feature linear Pattern one element dissapears

I have a feature pattern of 4 instances of the part inside a cutout in a part., and when I am making the pattern I see all 4, but when I OK the pattern the 4 vanishes. If I set the instance count to 5, it skips that one (skip is not checked)
Here is after hitting the green check:
I have the timeline rolled to the pattern, so there is no weird boolean afterwards if set to new I get 3
HOWEVER, if I set it to add, selecting the case around it:
Normally Add is the one that is trickier since it tends to trip up on the boolean part. Ideas as to what is wrong? I am on MacOS in Chrome (latest) on OS 26.0.1

After you click ok, Have you looked to make sure no parts are turned off?
