Onshape will not extrude

Any idea why this sketch wont extrude, its fully enclosed and presents no issues for face selection of shaded areas, i can extrude the inverse of its features?

Likely some nonmanifoldness. Could you share the document?

Another useful tip: If you are trying to debug a failure like the above, try switching from "Add" to "New". An "Add" operation is (under the hood) a "New" followed by a boolean union.

If the "new" operation succeeds: Problem is in the boolean. There's probably some sort of weird condition between the existing body and the one you just created. It doesn't isolate the problem - but it does tell you where to look (start and end conditions etc). Once you fix the boolean problem you can switch it back to an "Add".
If the "new" operation still fails: Then it's probably as @_anton says and is a problem with the sketch itself or the extrude operation resulting in non-manifold geometry.
