Revolve not working

I'm currently trying to make a revolve work, but it is saying "Revolve did not regenerate properly: Revolve would create self-intersecting part." How can I fix this? Image for issue:

    The image is not helpful - a link to the doc would have been better, but it looks like some of the vertices in the sketch are not coincident and if you selected a revolve axis other than one of the 4 outermost edges then it would intersect itself.
